Your First Visit
We can’t wait to welcome you for your first appointment! Upon arrival, you’ll be greeted by our friendly front desk team, who can assist you with any paperwork if needed. Please plan to arrive about 10 minutes early to complete the necessary forms. Don’t forget to bring details about your medical history, dental insurance, and any medications you’re taking. To save time, you can also fill out the forms in advance (see attachments below).
Financial Policy
As a courtesy to our valued patients, we’re pleased to offer direct insurance billing with most providers. Please remember that your insurance policy is an agreement between you, your employer, and the insurance company. We ask that you keep us updated on any policy changes to help us maximize your dental benefits.
Appointment Policy
We strive to provide high-quality care and personalized treatment in a timely manner. Each appointment is reserved specifically for you and your needs. To maintain smooth scheduling and accommodate all patients effectively, we request at least two business days' notice if you need to reschedule. We understand that emergencies can arise, and we’ll handle these situations on an individual basis.
